Fees for Service:

Diagnostic Evaluation:  $150.00 per 50 minutes

Individual, Couples, Marriage and Family therapy: $100.00 per 50 minutes

Legal Letters and other documentation:  $100 and up

Legal Deposition and Court Testifying:  $500  and up per hour

*Contractual rates apply when using insurance

 

Billing, Insurance, and EAP Verification:

I will work with you to verify your insurance, obtain managed care referrals, and will bill your insurance directly.  You are responsible for all deductibles, copayments, and any other amount not covered by insurance at the time of your appointment.  If you are going to use your Employee Assistance Program (EAP), you must bring with you to the first appointment the EAP name, telephone number, and authorization number.

Payments:

Robert accepts cash, VISA , MasterCard, American Express, and a personal check.   There is a $30.00 fee for any check that does not clear the bank for insufficient funds.  Should that occur, cash or a cashier check will only be accepted.
